Description:  Position: Licensed Physical Therapist  Qualifications:     Minimum of Bachelor of Science or Bachelor of Arts degree(minimum) to Doctorate degree from accredited college/university Physical Therapist Certification Minimum of 2 years public school experience preferred General Summary: The Physical Therapist will plan, evaluate, assess, implement, and provide physical therapy services to students in accordance with IEP goals and objectives. This position provides assistance to multiple teachers and building principals.  Flexibility and strong organizational skills are important.  It is equally important to be goal driven and maintain a high level of professionalism and confidentiality while adhering to Board Policy. Performance Responsibilities: Meet standards in knowledge and competence of physical therapy Meet standards in knowledge and competence in the areas of program implementation through appropriate service delivery options, including but not limited to: consultation, direct intervention, and collaboration Adheres to ethical standards of the physical/occupational therapy profession Tests and evaluates students for therapy and support needs Monitors, observes and evaluates student progress, design treatment plans and recommend programs suited to individual potential and rehabilitative needs based on physician’s orders Coordinate physical therapy with other therapeutic activities according to student needs, capabilities, interests, treatment goals and objectives Prepare and maintain a variety of records and files indicating student progress, care plans and related statistical data Provide instruction, guidance and technical assistance in physical therapy methods and techniques to Physical Therapy Assistant Schedules and conducts physical therapy for the students who require services Plans and carries out the IEP as part of the interdisciplinary team Consults with students, parents, and professionals concerning progress in physical therapy services Keeps records, writes reports and carries on clinical correspondence concerning the student's physical therapy Attends eligibility reviews and IEP meetings (as needed or requested by members of IEP team) Completes IEP paperwork for students Provides information concerning the nature, purpose, and outcome of physical therapy services and activities to incorporate at school and/or at home, to students, parents, and professionals  Document Medicaid forms and invoice for appropriate students monthly. Perform other duties as assigned by the Executive Director of Student Services  Physical Requirements/Environmental Conditions:  Requires the ability to manually move, lift, carry, pull or push heavy objects or persons, stoop, bend, and reach. Requires the ability to drive between schools in the district and regular, consistent attendance and physical presence at the job.
