| Description: | Transition Coordinator: The Barton, Dade, Jasper County Special Education Cooperative is seeking a transition coordinator for the 22-23 school year. The transition coordinator performs a variety of duties in providing appropriate services to special education students preparing for life after graduation. The transition coordinator will with students 16 and older to assist with individualized transition plans specific to interests and career options. The successful candidate must have strong interpersonal communication skills, student and parent interviewing skills, and be able to complete documentation for reporting and monitoring responsibilities.
